Sparta Community Hospital’s At-Home Health Care Administrator, Cheryl Adams, BSN, MBA, provided education at the recent Illinois Critical Access Hospital Network (ICAHN) Annual Conference. ICAHN’s mission is to strengthen critical access and small rural hospitals through collaboration.
Adams’ message, to those in attendance, was how to ensure that they have developed a plan for transitioning a patient through the different levels of care that will assist them in reducing rehospitalizations and ER visits, thus impacting their stance in Value Based Purchasing, a program that ultimately improves treatment outcomes for the United States patient population.
“Utilizing home health as a transition is one of the options.” states Adams. “All home health clinicians have the ability to provide consistent care and support patients through personalized experiences. I learned early on that home health clinicians have the power to be a lifeline for patients in more ways than I could have imagined. Patients depend on us, and after decades of life changing experiences in home care, I can’t think of a better place to fulfill the purpose of what led me into nursing in the first place: caring for people”.
